Chief Complaint:  A 62-year-old man with the history of PTC and total thyroidectomy.  Refer for evaluation due to increasing of serum thyroglobulin level

Present Illness  A 62-year-old man  One years ago (1401.04.02) he notices a lump in his neck.  He refer to the doctor due to that lesion. The doctor recommended an ultrasound.
